Eligibility





        Employees who work 30 hours or more per week and are non-union are eligible for all benefit
        plans. Part-time employees who work between 20–29 hours per week are eligible for medical,
        dental and vision benefits at part-time weekly premiums. Benefits are effective on the first day
        of the month following 60 days of employment. Benefit coverage will end on your last day of
        employment and you will be offered continuation of coverage through Cobra if elected.

        You may also enroll your eligible dependents for coverage. This includes the following:
        •  Your legal spouse or qualified domestic partner. *
        •  Your children, step-children, children of your domestic partner under the age of 26, regardless of student, dependency or
            marital status.
        •  Your children, step-children, children of your domestic partner who are over the age of 26 and are fully dependent on you
            for support due to a mental or physical disability, and who are indicated as such on your federal tax return.
        •  Per ERISA Plan Compliance, DPR requests proof of dependent relationship for dependent coverage. If you plan to cover
            dependents on your medical, dental or vision plan please come prepared with dependent documentation to prove
            dependent relationship. When adding a dependent to your coverage, you are certifying that they are an eligible dependent.
            Ineligible dependents cannot be added to our plans and are subject to audit and repayment.

        * If you enroll your domestic partner, contributions made by DPR towards the monthly premium for your domestic partner are subject to
        imputed income.
        Qualified Life Events

        You may change your benefit elections during the year if you experience a Qualified Life Event. Documentation will be required
        for all benefit changes due to a qualifying event.

        Change Benefits After Enrollment / Qualified Life Events


        During the year, you cannot make changes to your Medical, Dental or Vision coverage unless you have a Qualified Life Event.
        If you do not complete your Qualifying Life Event within 30 days of the Qualified Life Event, you will have to wait until the next
        annual Open Enrollment period to make changes (unless you experience another Qualified Life Event). Documentation will be
        required for all benefit changes due to qualifying event.

        DPR will periodically conduct audits to verify dependent status for those dependents enrolled in the DPR plans.
