Plans and Summary Plan Description
In addition, you and your dentist are responsible for choosing the course of treatment for (or
for choosing not to treat) any illness, injury or other dental condition. The Board and/or the
Nationals are not in any way responsible for the outcome of any dental treatment or care (or
lack of such treatment or care).
You should refer to the benefit booklets distributed to you to answer specific coverage
questions and to help you decide which options (if more than one option is available to you)
are right for you and your family. Copies of these benefit booklets are also available from
the Nationals.
Vision Coverage
The Nationals offer the vision coverage options described in Appendix D. You should refer
to the benefit booklet distributed to you to answer specific coverage questions and to help
you decide which options (if more than one option is available to you) are right for you and
your family. Copies of the benefit booklet are also available from the Nationals.
Life Insurance
The Nationals offer basic and supplemental life insurance coverage, which is described in
Appendix D. You may also purchase coverage for your eligible dependents. In addition, the
Nationals provide a basic level coverage in the event of loss as a result of an accident that is
equal to your basic life insurance benefit. You may purchase supplemental accidental death
and dismemberment insurance up to the amount of your supplemental life insurance. You
should refer to the benefit booklet distributed to you to answer specific coverage questions
and to help you decide which options (if more than one option is available to you) are right
for you and your family. Copies of these benefit booklets are also available from the
Nationals.
Disability
The Nationals provide income replacement benefits if the event that you become unable to
work for a period longer than 90 days. For specific information about the level, the duration,
and the limitations, please consult Appendix D, and the benefit booklet distributed to you by
the Nationals.
Flexible Spending Accounts
The Nationals sponsor a Flexible Benefit Plan that is intended to comply with the
requirements of Code section 125. Under this benefit, you may elect to pay the premiums for
your group health benefits on a pre-tax basis. You may also elect to participate in the Health
Care Reimbursement Account and/or the Dependent Care Assistance Account, programs that
allow you to use pre-tax dollars to pay for eligible medical or dependent care expenses,
respectively. Details of this program are in Appendix E.
