Plans and Summary Plan Description
INTRODUCTION
The Board of Trustees of the Major League Baseball League-Wide Insurance Program (the
“Board”) maintains the Major League Baseball League-Wide Insurance Program (the
“LWIP”) to provide health benefits for eligible employees and dependents of the following
entities (the “Baseball-Related Entities”):
Boston Red Sox (The Boston Red Sox Baseball Club, L.P.)
Houston Astros (Houston McLane Company, Inc.)
Lerner Corp.
Miami Marlins (Miami Marlins, L.P.)
Milwaukee Brewers (Milwaukee Brewers Baseball Club, L.P.)
Minnesota Twins
New York Mets (Sterling Mets, L.P.)
Oakland Athletics (Athletics Investment Group, LLC)
Pittsburgh Pirates (Pittsburgh Associates)
San Diego Padres (Padres, L.P.)
San Francisco Giants (San Francisco Baseball Associates L.P.)
St. Louis Cardinals (St. Louis Cardinals, LLC)
Tampa Bay Rays (Tampa Bay Rays Baseball, Ltd.)
Texas Rangers (Rangers Baseball Express, LLC)
Toronto Blue Jays (Rogers Blue Jays Baseball Partnership)

The Washington Nationals Baseball Club, Inc.1 maintains the Washington Nationals Baseball
Club, LLC Dental Plan, Washington Nationals Baseball Club LLC Group Life Insurance and
Group Long Term Disability Insurance and Vision Plan, the Washington Nationals Baseball
Club LLC Short Term Disability Benefits Program, and the Washington National Baseball
Club, LLC Section 125 Plan. Together, these benefits, are referred to in this booklet as the
“Nationals Plans.” The Nationals Plans and the LWIP are referred to collectively in this
booklet as the “Plan.”
This document serves two important functions related to the Plan under the Employee
Retirement Income Security Act of 1974 (“ERISA”), a federal law applying to employee
benefit plans. First, ERISA requires that employers must supply employees with a
description of the various benefit plans it maintains. The information must be included in a
summary plan description (“SPD”) for each plan. Second, ERISA requires that employee
benefit plans be maintained pursuant to a written plan document. This document, together
with the contract, benefit booklets, and other descriptive materials you have received from
the Board, the Nationals, insurance companies, and other organizations administering
