SUPPLEMENTAL LIFE and ACCIDENTAL

        DEATH & DISMEMBERMENT (AD&D)


        INSURANCE



         You  have  the  opportunity  to  purchase  Supplemental  Life  Insurance  for  yourself,  your  spouse,  and
         dependent children.

         Your cost for supplemental life insurance is based on the amount you elect and your age. You must purchase Supplemental
         Life Insurance for yourself in order to purchase spouse and/or dependent child (ren) coverage.

         Supplemental  AD&D  amount  is  equal  to  the  Supplemental  Life  coverage  you  elect  on  yourself,  your  spouse,  and
         dependent children. This program provides benefits when death is caused by an accident. It also provides accidental
         dismemberment and paralysis coverage. Benefits under the Supplemental AD&D plan are paid in addition to the benefits
         paid under the Supplemental Life Insurance policy (if purchased).

         Important: Evidence of Insurability (a brief health questionnaire) is required for all coverage above the Guaranteed
         Amounts, or if coverage is elected after your initial eligibility period







              Family Medical Leave Act (FMLA)

              If  you  have  been  with  the  company  for
              12  months,  you  may  be  eligible  for  up  to
              12  work  weeks  of  unpaid  leave  per  year
              under  the  Family  and  Medical  Leave  Act
              (FMLA). FMLA can be used for an illness of
              your own, care needed for a family member,
              care  for  a  newborn,  and  certain  other
              medical needs.
