Group Policy Effective Date: February 1, 2010
Policy Issued in: District of Columbia

Member means:

1. A regular L.L.C. Owner-Employee or non-seasonal employee of the Employer who is:
a. Actively At Work at least 35 hours per week (for purposes of the Member definition,
Actively At Work will include regularly scheduled days off, holidays, or vacation days, so
long as the person is capable of Active Work on those days); and
b. A citizen or resident of the United States or Canada; or

2. A regular non-seasonal coaching or training employee of the Employer who is:
a. Actively At Work at least 35 hours per week (for purposes of the Member definition,
Actively At Work will include regularly scheduled days off, holidays, or vacation days, so
long as the person is capable of Active Work on those days); and
b. A foreign national under contract with the Policyholder working (or performing services)
in the United States.
Member does not include a temporary or seasonal employee, a full-time member of the armed
forces of any country, a leased employee, or an independent contractor.
Class Definition:
Class 1: All Members with Annual Earnings of $90,000 or more

Class 2: All Members with Annual Earnings less than $90,000

SCHEDULE OF INSURANCE

Eligibility Waiting Period: You are eligible on the first day of the calendar month
coinciding with or next following 30 consecutive days as a
Member.
Eligibility Waiting Period means the period you must be a Member before you become eligible for
insurance.

Own Occupation Period: Class 1: From the end of the Benefit Waiting Period to the
end of the Maximum Benefit Period.
