Please be advised that the Accident/Incident Report form has been updated as an electronic file for all project
teams’ use.
• In the event of an accident or incident involving an injury and/or property damage, please use the
Accident/Incident Report form when reporting the event to the Safety Committee. The attached PDF is a
fillable form for your convenience.

We hope for a minimal use of this form; however, this will help all of our project teams’ recordkeeping in case of an
incident/accident.
