Heritage School of Texas is committed to a comprehensive benefit program that helps you stay healthy and
feel secure while maintaining a work/life balance.

Heritage School of Texas is pleased to continue offering two (2) Baylor Scott & White Medical plan options
to meet your individual needs. Both are Nationwide PPO Plans with in and out of network coverage. We also
offer our full-time employees Voluntary Dental and Vision through Humana. Heritage School of Texas pays a
sizable flat dollar amount of each full-time employee’s cost for the medical, dental or vision plan of your choos-
ing.

Heritage School of Texas is concerned about your financial security and for that, we offer an option to enroll in
Voluntary Short Term Disability (STD) and Voluntary Long-Term Disability (LTD). We also provide all Full-Time
employees with a flat amount of $25,000 Basic Life insurance with Accidental Death and Dismemberment
(AD&D).

Navigator Login Instructions on page 19. Note that this booklet does not function as an ID card or proof of benefits for
