          Apollo  Healthcare  Consulting  and  Management  is  committed  to  a  comprehensive  benefit
          program that helps our employees stay healthy and feel secure while maintaining a quality work /
          life balance.



          Apollo Healthcare Consulting and Management  is pleased to offer our full-time employees four (4)
          United  Healthcare  (UHC)  Major  Medical  Options  and  two  Pan  American,  Limited  medical  lower
          cost  options  with  Healthcare2U  (DPC)  Direct  Primary  Care  benefit  that  offer  Unlimited  $10  PCP
          Doctor Visits and Unlimited $25 Urgent Care visits, see pages 15 & 16 for more information. Both UHC
          and  Pan-American  have  Unlimited Preventative  Care coverage  with  NO  Copay,  Deductibles,  or
          Out of Pocket Costs. Apollo Healthcare Consulting and Management  contributes a portion of the
          monthly  employee only premium for all of the Medical plan options.

          Beginning May 1st, we are offering an ALL NEW ancillary package from Mutual of Omaha. The NEW
          offering will include two Voluntary Dental plan options and a Voluntary Vision plan as well as low
          cost  Voluntary  Employee  Life  Insurance  with  matching  Accidental  Death  &  Dismemberment.
          Please Note ALL employees and New Hires (during initial enrollment) can apply for up to $150,000
          with  NO  Health  Questions,  Spouses  $30,000  and  Children  $10,000.  Also  new  this  year  are  Critical
          Illness  with  NO  Health  Questions  meaning  you  cannot  be  turned  down,  Accident  Coverage  for
          employees and dependents, and Voluntary Short-Term Disability (STD).


          Please retain a copy of this guide in order to review coverage benefits, cost information, and retain
          necessary contact information. This booklet will not serve as an ID card or verification of benefits at
          a provider’s office, but can provide assistance if needed. Refer to page 33 for instructions on how
          to enroll in benefits. For additional contact numbers and information please refer to page 34.
