major medical plan options and two Pan American plan options with a NEWLY added
Healthcare2U Direct Primary Care benefit that offers Unlimited $10 PCP Doctor Visits and Unlimited
$25 Urgent Care visits, see pages 10-11 for more information. Our benefit package also includes
Voluntary Dental, and Vision
Revelations Healthcare Group contributes a portion of the monthly employee only premium for all
of the Medical plan options.
Please retain a copy of this guide in order to review coverage benefits, cost information, and retain
necessary contact information. This booklet will not serve as an ID card or verification of benefits at
a provider’s office, but can provide assistance if needed. Refer to page 19 for instructions on how
