North Texas Neuroscience and Sleep Center, P.A. is committed to a comprehensive benefit
program that helps our employees stay healthy and feel secure while maintaining a work/
life balance.

North Texas Neuroscience and Sleep Center, P.A. is pleased to offer you two medical
options to meet your individual needs. We also provide our employees with voluntary
dental, vision, life/ AD&D, and employer paid employee vision, LTD, and life/AD&D.

North Texas Neuroscience and Sleep Center, P.A. pays the majority of the cost of the
employee’s medical plans. Our employees have the option to purchase medical
coverage for their spouse and dependents. We also pay 100% of your vision and basic life
and AD&D. We provide optional dental and vision for our employees, their spouses, and
children.

Please retain a copy of this guide in order to review benefits, cost information, and contact
information. This booklet will not serve as an ID card or verification of benefits at a providers
office, but can provide assistance if needed.
