FW Specialty Surgicare is committed to a comprehensive benefit program that helps our
employees stay healthy and feel secure while maintaining a quality work/life balance.
FW Specialty Surgicare is pleased to offer all our full-time employees two Major Medical plans from
Blue Cross Blue Shield of Texas including a BASE HMO plan and a Buy-Up PPO option.
The Blue Cross Blue Shield of TX (BCBS) Medical plans both come with 100% paid Preventive Care
coverage with NO Copay, Deductibles, or Out of Pocket Costs.
FW Specialty Surgicare contributes a portion of the monthly employee only premium for all of the
Medical plan options. You are welcome to include your dependents in your medical coverage.
Please retain a copy of this guide in order to review coverage benefits, cost information, and retain
necessary contact information. This booklet will not serve as an ID card or verification of benefits at
a provider’s office, but can provide assistance if needed.
