         Megatel Homes, LLC is committed to a comprehensive benefit program that helps our employees stay healthy and feel
         secure while maintaining a quality work/life balance.

         MEDICAL is renewing with United Healthcare (UHC) but changing to Level Funded Option with new plans that mirror that current designs as close
         as possible. Our medical plans offer a variety of Doctor Copays, Deductibles, and Out of Pocket Maximums. (Medical plans come with new Virtual
         Visits through HealthiestYou.  Virtual Visits cover a wide range of services including Mental Health. There is a wide range of Wellness Benefits to
         including UHC Rewards, where you’re able to earn up to $1,000 for completing health and wellness activities (refer to page 9). Megatel Homes
         contributes Over 74% of Employees Cost and ,we contribute up to 56% for Employees (with Dependents) depending on the plan you choose.

         VOLUNTARY  DENTAL  is renewing with United Healthcare. Both dental plans provide 100% preventative services with annual deductible and
         calendar year maximum waived. Both plans include MaxMultipiler benefit, which is a way to increase your annual maximums. The Buy Up Plan
         now has “true” Implant coverage. United Healthcare has a much larger Dental PPO network. Buy-Up Dental plan reimbursement is based on usual
         and customary level as well.

         VOLUNTARY  VISION  is renewing through United Healthcare too. In addition to major vision providers such as Walmart, Costco, Target, Lens
         Crafters  you have access to Warby Parker  Network with complete set of glasses for only a $25 Copay.

         VOLUNTARY LIFE and AD&D  is offered through Lincoln Financial for employees and dependents. Employees for 2025 OPEN ENROLLMENT
         and NEW  HIRE period have Guaranteed Issue, NO health questions up to $150,000 for employees, $50,000 for Spouses and $20,000 for Child
         (ren). Voluntary life insurance includes Accidental Death and Dismemberment (AD&D) for employees and spouses. Any amounts you already have
         over these amounts will be grandfathered.

         VOLUNTARY STD and LTD is with Lincoln Financial that has LOWER COSTS this year for our Short Term Disability starting 15th day WITH No
         Health Questions. You also have ability to add Long Term Disability starting after 90 days covering 60% of your Income Tax Free! LTD benefit peri-
         od up to age 65 of Normal Social Security Retirement Age.

         VOLUNTARY  ACCIDENT  PLAN  Lincoln Financial is a Low Cost comprehensive accident coverage for fractures, burns, lacerations, hospital,
         surgical, ER and doctors, accidental death and dismemberment.

         NEW CRITICAL ILLNESS  through Lincoln Financial has 100% lump sum benefits for Cancer, Stoke, Health Attack, Major Organ Failure  & many
         other critical illness covered at 100%

         Our benefit guide will not serve as an ID card or verification of your benefits at a provider’s office, but it can provide assistance if needed.
