TriTrax Rehabilitation is committed to a comprehensive benefit program that helps our
employees stay healthy and feel secure while maintaining a work/life balance.
TriTrax is pleased to offer dual medical options, voluntary-dental, vision, life and disability
coverage to our employees and their dependents. We also continue to provide 100%
employer paid life and AD&D for our employees. TriTrax Rehabilitation covers 54% of
either medical plans for the employees only premium to make this more affordable.
For more information 24/7 please see your Employee Benefit Website for all your benefit
information needs. Employee Navigator benefit portal includes your plan information &
summaries, costs, carriers contact information, and network provider look up. You can al-
so find helpful (how to) videos, provider links, and forms.
Log in and instructions can be found on pages 20-21 for instructions on how to log into
Employee Navigator and choose your benefits.
Please retain a copy of this guide in order to review coverage benefits, cost information,
and necessary contact information on page 22. This booklet will not serve as an ID card
or verification of benefits at a provider’s office, but can provide assistance if needed.
