Basic Life Insurance and Accidental Death & Dismemberment (AD&D)
Affinity Neurocare provides all full-time employees a life insurance benefit and matching AD&D benefit base
on your class. We pay 100% of the cost of this benefit. Your benefits will reduce 35% at age 65, an additional
15% at age 70, and an additional 15% of the original amount at age 75, coverage terminates upon employ-
ees retirement Conversation option is available.
Voluntary Life Insurance and Accidental Death & Dismemberment (AD&D)
Affinity Neurocare offers all full-time employees Voluntary Life for you, your spouse, and your dependent children
at low costs term rates. Accidental Death and Dismemberment is included at no additional cost for employee
and spouse coverage. This is 100% employee paid and will be paid through payroll deduction. In order to elect
coverage for a spouse or child, you must elect coverage for yourself. Your benefits will reduce to 65% at age 65,
and another 15% at age 70. Spouse’s benefits terminate at Employee’s age of 70 or retirement,
whichever comes first.
