          Citizens Bank of Ada is committed to a comprehensive benefit program that helps our
          employees stay healthy and feel secure while maintaining a work/life balance.

          Citizens Bank of Ada is pleased to meet your individual needs by offering you medical,
          dental, vision, basic life and AD&D, long-term disability and voluntary life and AD&D.
          Citizens Bank of Ada’s NEW Blue Cross Blue Shield of Oklahoma (BCBSOK) Medical plan.
          Both  virtual  doctor  visits  and MDLive  24/7  Telemedicine  have $0 Copays.  We  offer  an
          Employee Assistance Program (EAP) for all employees and dependents.

          Citizens Bank of Ada pays the majority of the cost of the employee’s medical and 100%
          of  dental  based  on  your  medical  election.  Our  employees  also  have  the  option  to
          purchase  dental,  vision,  and  voluntary life  coverage for  themselves,  their  spouse,  and
          children without medical. We pay 100% of your long-term disability and basic life and
          AD&D.

          2023 Open Enrollment and New Hires will have the option of guaranteed issue amounts
          this year for voluntary employee life up to $100,000, voluntary spousal life up to $30,000,
          and  voluntary  child  life  of  $10,000.  This  means  NO  Health  Questions  and  cannot  be
          turned down.

          Please retain a copy of this guide in order to review benefits, cost information, and con-
          tact information.  This booklet will not serve as an ID card or verification of benefits at a
          provider’s office, but provides a basic summary of benefits available to you. For more
          information and to review the Glossary of Health Coverage and Medical Terms, log into
