         Wesco Refractories, Inc. is committed to a comprehensive benefit program that helps our employees stay
         healthy and feel secure while maintaining a quality work/life balance.

         MEDICAL We offer two comprehensive medical plans through Blue Cross and Blue Shield of Texas (BCBS).
         Both plans are HMO plan which require you have an assigned Primary Care Physician (PCP) and get refer-
         rals for Specialists. Our medical plans offer the option of a $3K or $6K Calendar Year Deductible (CYD) and
         low copays. Virtual Visits through BCBS are powered by MDLIve and there are numerous wellness and fit-
         ness programs included at no additional charge.

         DENTAL  We are continuing to offer our comprehensive Dental coverage through Humana. Our Dental plan
         has $1,500 annual maximum benefits, plus 30% coverage after the $1,500. Our plan reimburses non-network
         dentists at the highest usual and customary level, giving you freedom  to  choose  and dentist. Preventative
         services are covered 100% with no deductible.

         VISION We are also continuing to offer our comprehensive Vision coverage through Humana. Our plan offers
         Exams, Frames, low cost copays for covered lens options, and a $130 allowance for Frames or Contacts in
         lieu of glasses.

         (Employee Navigator), anytime 24/7. This personalized portal offers detailed information on your current
         plan, benefit summaries, cost breakdowns, provider and network search tools, instructional videos, carrier
         links, and relevant documents.


         To access and navigate the Employee Navigator, please refer to pages 21-22 of this guide for detailed login
         instructions and benefit selection procedures.

         Please retain a copy of this guide for future reference, including coverage benefits, cost information, and es-
         sential contact details provided on page 23. Note that this booklet does not function as an ID card or proof of
         benefits for providers but can assist you with relevant information as needed.
