that helps our employees stay healthy and feel secure while maintaining a

Frank Bailey Grain Co. is pleased to offer you a medical option to meet
your individual needs. Frank Bailey Grain includes an Health Reimbursement
Arrangement HRA to reimburse employees and dependents after you
reach $1,250 of your calendar year deductible. We also provide our
employees voluntary dental and employer paid life and AD&D.
Frank Bailey Grain Co. does cover a major portion of the medical cost to
make this more affordable for our employees. We also pay 100% of your
basic life and AD&D coverage.
Please retain a copy of this guide in order to review coverage benefits,
cost information, and retain necessary contact information on page 14. This
booklet will not serve as an ID card or verification of benefits at a providers
office, but can provide assistance if needed.
