          SkyBlue Healthcare is pleased to offer all our full-time employees 3 PPO and 3 HMO Blue Cross  Blue
          Shield  of  Texas  Major  Medical  Options  and  two  NEW  Pan  American  Limited  medical  lower  cost
          options with direct access to a network of Primary Care Doctors and virtual telehealth services at no
          costs. BOTH Pan American & Blue Cross Blue Shield of TX (BCBS) comes with 100% paid Preventative
          Care coverage with NO Copay, Deductibles, or Out of Pocket Costs. NEW this year we including
          $10,000  Employer  Paid  Life  Insurance  and  matching  AD&D  with  Principal.  Our  benefit  package
          includes  Voluntary  Dental,  and  Vision  with  Principal.  We  also  offer  low  cost  Voluntary  Life  with
          matching  Accidental  Death  &  Dismemberment.  4-1-24  Open  Enrollment  has  $150,000  Life/AD&D
          there are NO Health Questions, Spouses $30,000 and Children $20,000.  Skyblue also offers Voluntary
          Critical Illness that has NO Health Questions meaning you cannot be turned down.       We offer On
          and Off the Job Accident Coverage for employees and dependents and Hospital  Indemnity Insur-
          ance, Voluntary Short-Term Disability (STD) through Principal.

          SkyBlue Healthcare, LLC contributes a portion of the monthly  employee only premium for all of the
          Medical plan options.

          Please retain a copy of this guide in order to review coverage benefits, cost information, and retain
          necessary contact information. This booklet will not serve as an ID card or verification of benefits at
          a provider’s office, but can provide assistance if needed. Please refer to page 28-29 for Employee
          Navigator instructions on how to enroll in benefits. For additional contact numbers and information
