Frank Bailey Grain Co. is committed to a comprehensive benefit program
that helps our employees stay healthy and feel secure while maintaining a
work/life balance.
Frank Bailey Grain Co. is pleased to continue offering you a medical option
that meets your individual needs. Our UHC major medical policy has a low
$2,500 deductible for individuals and $7,500 for Family. We have two volun-
tary Humana Dental plans to choose from and we also provide our em-
ployees with a minimum of $20K in employer paid life insurance and AD&D.
Frank Bailey Grain Co. does cover a major portion of the medical cost to
make this more affordable for our employees. We also pay 100% of your
basic life and AD&D coverage.
Please retain a copy of this guide in order to review coverage benefits,
cost information, and retain necessary contact information on page 12. This
booklet will not serve as an ID card or verification of benefits at a providers
office, but can provide assistance if needed.
