Accidental Death & Dismemberment (AD&D)

Brown, PC provides full-time employees a life insurance benefit of $50,000 and an AD&D
policy equal to the life insurance benefit. Brown, PC pays the full cost of this benefit.
Please note your Benefits will reduce 35% at age 65, and an additional 35% of original
amount at age 70. Benefit Terminates at retirement.

Included: If it is determined that you are totally disabled, your life insurance

Beneficiary Assist include: Legal advice, financial planning and emotional/grief counseling: up
to five face to face sessions or equivalent professional time for one or a
combination of available services.

Everest Funeral funeral planning needs, a detailed pricing comparison of local funeral homes,
Planning concierge services at or near the time of death, including cost negotiation,
and claims payments in as little as 48 hours.
