SkyBlue Healthcare is pleased to offer all our full-time employees Three United Healthcare Major
Medical Options and two Pan American Limited medical lower cost options with HealthiestYou
benefit that offers virtual telehealth services. UHC has Unlimited Preventative Care coverage with
NO Copay, Deductibles, or Out of Pocket Costs. Our benefit package includes Voluntary Dental,
and Vision with Guardian. We also offer low cost Voluntary Life with matching Accidental Death &
Dismemberment. Please Note New Hires (during initial enrollment) can apply for up to $100,000 with
NO Health Questions, Spouses $25,000 and Children $10,000. We also offer Voluntary Critical Illness
with NO Health Questions meaning you cannot be turned down. On and Off the Job Accident
Coverage for employees and dependents. Hospital Indemnity Insurance, Voluntary Short-Term
Disability (STD). SkyBlue Healthcare contributes a portion of the monthly employee only premium
for all of the Medical plan options.
Critical Illness and Hospital Indemnity are not available in all locations.

necessary contact information. This booklet will not serve as an ID card or verification of benefits at
a provider’s office, but can provide assistance if needed. Refer to page 28-29 for Employee Navi-
