(BCBS) Major Medical Option for yourself and your family. Our plan is on the BCBS Blue Choice (PPO)
Nationwide network and offers In and Out of Network coverage.
Mara’s Med Spa contributes a large portion of the monthly employee only premium for our employees.
For more information 24/7 please see your customized Employee Navigator portal for all your benefit
information needs. Your Employee Navigator benefit portal includes your plan information, summaries,
costs, carrier contact information, and network provider look up. You can also find helpful videos,
provider links, and forms.
Please retain a copy of this guide for future reference, including coverage benefits, cost information,
and Employee Navigator Login Instructions on pages 10 & 11 Note that this booklet does not function
as an ID card or proof of benefits for providers but can assist you with relevant information as needed.
