Sky Blue Healthcare, LLC is committed to a comprehensive benefit program that helps our employees stay healthy and
         feel secure while maintaining a quality work/life balance.

         SkyBlue is pleased to offer all our full-time employees Major Medical plan options through Blue Cross Blue Sheild of
         Texas. We have a NEW HMO plan with the larger Blue Essentials Network and 3 PPO options under the Blue Choice
         Network with in and out of network coverage. We are continuing to offer two low-cost alternative LIMITED Medical plan

         options through Pan American Life. These plans include direct access to a network of Primary Care Doctors (PCP) and
         virtual telehealth services at no costs when using HealthCare2U. BOTH Pan American & Blue Cross Blue Shield of TX
         (BCBS) come with 100% Paid Preventative Care coverage with NO Copay, Deductibles, or Out of Pocket Costs.

         All full-time employees are enrolled in $10,000 of Employer Paid Life Insurance and matching AD&D with Principal. Our
         benefit package also includes Voluntary Dental, Vision, Short Term Disability (STD) and low-cost Voluntary TERM Life
         with matching AD&D. New Hires can enroll up to $150,000 (Guaranteed Issue) of Voluntary Life/AD&D with NO Health
         Questions asked. Spouses up to $30,000 and Children up to $20,000.  Currently enrolled employees can increase by one
         to two units ($10,000-$20,000) during Open Enrollment without health questions.
         We are now also offering several NEW lines of coverage with TransAmerica Life, including Accident Coverage for em-
         ployees (and dependents), Hospital- Indemnity Insurance, Critical Illness with NO Health Questions (meaning you can-
         not be turned down), and UNIVERSAL Life Insurance with Guaranteed Issue up to $75,000.



         ALL PLANS ARE OFFERED TO EMPLOYEES AND YOUR DEPENDENTS STAND ALONE SO YOU CAN CHOOSE
         WHAT MEETS YOUR NEEDS. Sky Blue Healthcare, LLC contributes a large portion of the monthly employee only premi-
         um for all of the Medical plan options and 100% of the cost for the Basic ($10,000) Life Insurance coverage with Principal.



         Please retain a copy of this guide to review coverage benefits, cost information, and retain necessary contact information.
         This booklet will not serve as an ID card or verification of benefits at a provider’s office but can help if needed.



         THIS YEAR BenROLL Counselors will enroll you in Employee Navigator and go over your benefits and costs. After you
         have completed your enrollment, please refer to your Employee Navigator portal for documents and your benefit infor-
         mation. See page 30 for instructions on logging into Employee Navigator.
