Citizens Bank of Ada is committed to a comprehensive benefit program that helps our employees stay healthy and feel
secure while maintaining a work/life balance.
Citizens Bank of Ada is pleased to meet your individual needs by offering you medical, dental, vision, basic life and
AD&D, long-term disability and voluntary life and AD&D. Citizens Bank of Ada’s Blue Cross Blue Shield of Oklahoma
(BCBSOK) Medical plan includes both virtual doctor visits and MDLive 24/7 Telemedicine with $0 Copays. We also
offer an Employee Assistance Program (EAP) for all employees and dependents.
Citizens Bank of Ada pays the majority of the cost of the employee’s medical and 100% of dental based on your medical
election. Our employees also have the option to purchase dental, vision, and voluntary life coverage for themselves, their
spouse, and children without medical. We pay 100% of your long-term disability and basic life and AD&D.
New Hires will have the option of guaranteed issue amounts this year for voluntary employee life up to $150,000,
voluntary spousal life up to $30,000, and voluntary child life of $10,000. This means NO Health Questions and cannot be
turned down. 2024 During Open Enrollment if you are currently enrolled in Voluntary Life for yourself, you may
increase one increment of $10,000. No increases are allowed for Spouse.
