Twin City Gardens is pleased to offer our full-time employees four United Healthcare major medical
plan options and three Pan American plan options with that offers Unlimited annual preventa-
tive care benefits with NO out of pocket costs. Both plans offers Virtual Healthcare visits at $0 Co-
pay cost. Voluntary Dentary is offered thought Delta Dental and a NEW vision program with en-
hanced benefits Superior Vision.
Twin City Gardens pays a major portion of the employees cost for any of the medical options you
choose.
Please retain a copy of this guide in order to review coverage benefits, cost information, and retain
necessary contact information. This booklet will not serve as an ID card or verification of benefits at
a provider’s office, but can provide assistance if needed. Refer to page 22 for instructions on how
