Stamford Residence and Rehabilitation is committed to a comprehensive benefit program that
helps our employees stay healthy and feel secure while maintaining a quality work/life balance. We
offer a Enhanced EAP program for our employees and dependents.
Stamford Residence and Rehabilitation is pleased to offer our full-time employees four United
Healthcare major medical plan options. Navigate HMO plan with enhanced benefits where their
many services are covered with the calendar year deductible, waived. You also have two low cost
Limited Medical options through Pan American. ALL of our Medical Plans offer unlimited annual
preventative care benefits with NO out of pocket costs. ALL of our Medical Plans offer Virtual
Healthcare visits at $0 copay cost. Your employer pays a major portion of the employees and
dependent cost for any of the medical options you choose.
We offer Voluntary Dental through Mutual Of Omaha and Voluntary Vision through Superior Vision.
We provide 100% Employer Paid Life and Matching Accidental Death & Dismemberment for all full-
time employees. In addition to employer paid life we offer low cost group Voluntary Life with
matching Accidental Death & Dismemberment for employees and dependents through Mutual of
Omaha. During OPEN ENROLLMENT and New Hire Employees can enroll in $150,000 with NO Health
Questions and cannot be turned down. Dependents also have levels of Guaranteed Issue availa-
ble. If you are enrolled in voluntary life you can increase up to $20,000 with NO health questions.
We offer two comprehensive voluntary Accident Plans, one On and Off the job coverage and the
other Off the Job Only. Both have same benefits. We have voluntary Critical Illness coverage that
includes Cancer, Heart Attack, Stroke and much more coverage. Our plan is Guaranteed Issue
which means you cannot be turned down. Children are added at NO cost.
Please retain a copy of this guide in order to review coverage benefits, cost information, and retain
necessary contact information. This booklet will not serve as an ID card or verification of benefits at
a provider’s office, but can provide assistance if needed. Refer to page 34 for instructions on how
