VOLUNTARY Dental is with Ameritas (formerly Dental Select). We offer a PPO dental plan which provides
100% covered preventative services, 80% covered Basic Services and 50% Covered Major services with a
low annual deductible and a large calendar year maximum.

VOLUNTARY Vision is with Ameritas (formerly Dental Select). Vision through Ameritas uses the EyeMed
Select Network of providers and facilities and comes with $100 Frame Allowance and up to $115 for
contacts

EMPLOYER PAID Group Life and AD&D we provide all Full-Time employees with a flat amount of Em-
ployer Paid Life Insurance and Accidental Death and Dismemberment (AD&D) is included. Benefits in-
clude an Employee Assistance Program (EAP) and Emergency Travel Assistance.

EMPLOYER PAID Short-Term Disability (STD) provides all Full-Time Employees with Employer Paid
Short-Term Disability (STD) Insurance. Financial help to cover your expenses if you're injured or give birth.

EMPLOYER PAID Long-Term Disability (LTD). provides all Full-Time Employees with Employer Paid
Long-Term Disability (LTD) Insurance. Protection to help replace income in challenging times. Pays a por-
tion of your salary after you have been ill or injured for an extended period of time.

Navigator), anytime 24/7. This personalized portal offers detailed information on your current plan, benefit summaries,
cost breakdowns, provider and network search tools, instructional videos, carrier links, and relevant documents.
