         TriTrax Rehabilitation is committed to a comprehensive benefits program that helps our  employees
         stay healthy and feel secure while maintaining a work/life balance.

         TriTrax is pleased to offer dual medical options, voluntary-dental, vision, life and disability coverage
         to our employees and their dependents. We also continue to provide 100%   employer paid life
         and  AD&D  for  our  employees.  TriTrax  Rehabilitation  covers  50%  of  either  medical  plans  for  the
         employees only premium to make this more affordable.

         BCBSTX Medical HMO Blue Advantage Network Platinum Plan has the lowest Calendar  Deductible
         and Annual Out of Pocket. Does require Referral for Specialist including In & Out Patient Hospital.
         The Medical PPO Blue Choice Network Silver Plan has In and Out of Network Coverage and Does
         NOT require referrals.

         Dental is with Humana and has an UNLIMTED annual maximum benefit and out of network dentists
         and reimbursed at the Usual and Customary levels. Vision benefits offered through Humana.

         We provide $50,000 Life with Matching AD&D for all full-time employees at no cost. We also offer
         low cost term Voluntary Life and AD&D  with Mutual of Omaha.  This year is New Guaranteed Issue
         of  $100,000  for  Employees.  $25,000  for  spouses  and  $10,000  Children  (You  can  NOT  be  turned
         down)

         We offer voluntary Long Term Disability (STD) with Mutual of Omaha for all full-time employes.

         information needs. Employee Navigator benefit portal includes your plan information & summaries,
         costs, carriers contact information, and network provider look up. You can also find helpful (how
         to) videos, provider links, and forms.

         Log  in  and  instructions  can  be  found  on  pages  24  and  25  for  instructions  on  how  to  log  into
         Employee Navigator and choose your benefits.

         Please  retain  a  copy  of  this  guide  in  order  to  review  coverage  benefits,  cost  information,  and
         necessary contact information on page 26. This booklet will not serve as an ID card or verification
         of benefits at a provider’s office, but can provide assistance if needed.
