         Queen  Sweet  Jane,  LLC  is  committed  to  a  comprehensive  benefit  program  that  helps  our
         employees  stay  healthy  and  feel  secure  while  maintaining  a  quality  work/life  balance.  We  now
         offer a Enhanced EAP program for our employees and dependents.

         Queen Sweet Jane, LLC is pleased to offer our full-time employees three United Healthcare major
         medical plan options. You also have two low cost Limited Medical options through Pan American.
         ALL of our Medical Plans offer  unlimited annual preventative care benefits with NO out of pocket
         costs. ALL of our Medical Plans offer Virtual Healthcare visits at $0 copay cost.  Queen Sweet Jane,
         LLC pays a major portion of the employees and dependent cost for any of the medical options you
         choose.

         We offer Voluntary Dental through Mutual Of Omaha and Voluntary Vision through Superior Vision.

         This  year,  we  are  pleased  to  offer  our  NEW  100%  Employer  Paid  Life  and  Matching  Accidental
         Death  &  Dismemberment.  We  are  also  now  pleased  to  offer  low  cost  group  Voluntary  Life  and
         matching  Accidental  Death  &  Dismemberment  for  employees  and  dependents.  During  OPEN
         ENROLLMENT and New Hire Employees can enroll in $150,000 with NO Health Questions and cannot
         be turned down.  Dependents also have levels of Guaranteed Issue available.

         NEW this year through Mutual of Omaha are two comprehensive Accident Plans, one On and Off
         the job coverage and the other Off the Job Only. Both have same benefits.

         NEW this year through Mutual of Omaha is $10,000 to $20,000 Critical Illness coverage that includes
         Cancer,  Heart  Attack,  Stroke  and  much  more  coverage.  Our  plan  is  Guaranteed  Issue  which
         means you cannot be turned down. Children are added at NO cost.

         Please retain a copy of this guide in order to review coverage benefits, cost information, and retain
         necessary contact information. This booklet will not serve as an ID card or verification of benefits at
         a provider’s office, but can provide assistance if needed. Refer to page 34 for instructions on how
