helps our employees stay healthy and feel secure while maintaining a work/life balance.

Advanced Neuro Solutions offers a choice of two (2) comprehensive Major Medical plans
with Blue Cross Blue Shield of Oklahoma. Both Medical plans come with Virtual Doctor Visits
and comprehensive annual preventative care benefits covered 100% along with other
Wellness programs for healthier lifestyles. Major Network differences are explained below:

• The Blue Advantage Network is smaller and being offered as a Base plan since it is lower
in cost.

• The Blue Preferred Network is Oklahoma's largest PPO Network and will be offered as a
Buy-Up option for employees wanting a richer plan.

• Employees living outside of Oklahoma will only need to ensure they choose a provider in
the Blue Choice PPO Network. (More info on page 5)

We provide 100% Employer Paid Dental, Vision, Basic Life and AD&D for full-time employees.
Advanced Neuro Solutions also contributes a major portion towards your medical. Dental
NOW has Adult Orthodontics Included.

Our benefit guide will not serve as an ID card or verification of your benefits at a provider’s
office, but it can provide assistance if needed. Refer to page 18 for instructions on how to
