         Pump  Down  Specialist,  LLC  is  committed  to  a  comprehensive  benefit  program  that  helps  our  employees  stay  healthy  and  feel  secure  while
         maintaining a quality work/life balance.

         MEDICAL:  will continue with United Healthcare (UHC) and will continue to offer a nationwide comprehensive medical plan. Our medical plan in-
         cludes a variety of Doctor Copays, Deductibles, and Out of Pocket Maximums. Our UHC Medical plan come with $0 PCP visits for children under
         age 19 and $0 Virtual Visits for everyone. Pump Down Specialist LLC, covers 100% of the Employees Cost to enroll and you may cover your
         spouse and children at an additional cost. Medical Comes with Cash Care $200 for individuals and $500 for families.

         DENTAL:  our Principal dental includes a Calendar Year (CY) Maximum of $3,000  for each covered member. Dental plan provides preventative
         services paid at 100% with annual deductible waived. Dental plan includes enhanced Endodontic/Periodontic coverage of 80%, implant coverage,
         and Max Accumulator, which is a way to potentially increase your annual maximums by up  to  $2,000. Also, any out -of-network claims are pro-
         cessed at the 90th percentile of U&C, meaning you have flexibility to choose any dentist (In or Out of Network) with minimal impact.

         VISION:  Will continue with Principal has $10 Copay for exams and materials and a $150 Frame/Contact Lens allowance with 20% off anything
         over.

         Employer Paid Basic LIFE: Pump Down Specialist, LLC provides ALL  Full-Time employees with a 100% Employer Paid life insurance benefit of
         $100,000; including matching Accidental Death and Dismemberment (AD&D). Your benefits will reduce to 65% at age 65, and to 50% at age 70.
         Benefits terminate at retirement.

         VOLUNTARY LIFE and AD&D is offered through Principal Life for employees and dependents. Employees in their NEW HIRE Guaranteed Issue
         ($100,000  for  Employee  /  $25,000  for  Spouse  /  $10,000  for  Child(ren),  with  NO  health  questions  asked  and  Cannot  be  turned  down.
         ALL Employees can increase at up to two (2) increments ($10,000 Employee / $5,000 Spouse). With no health questions. Voluntary life insurance
         includes matching AD&D.

         VOLUNTARY  ACCIDENT  PLAN  with Allstate Benefits is a Low Cost, 24  Hour  (ON  &  OFF  the  Job)  comprehensive accident coverage for
         fractures, burns, lacerations, hospital, surgical, ER and doctors, accidental death and dismemberment.

         VOLUNTARY Critical Illness PLAN  with Allstate Benefits provides financial protection by providing a lump sum cash benefit if you’re diagnosed
         with life threatening illnesses such as: cancer, heart attack, stroke and many other major illnesses. No Evidence of Insurability required.
