         Megatel  Homes,  LLC  is  committed  to  a  comprehensive  benefit  program  that  helps  our  employees  stay
         healthy and feel secure while maintaining a quality work/life balance.

         MEDICAL is renewing with United Healthcare (UHC) and will continue to offer four comprehensive medical
         plans. Our medical plans offer a variety of Doctor Copays, Deductibles, and Out of Pocket Maximums.  (During
         the COVID-19 period, ALL Testing and Treatment is covered 100%. Members have NO cost share). Medical
         plans come with Virtual Visits, UHC has Employee Assistance Program (EAP) program and a number of Well-
         ness programs for healthier lifestyles including UHC’s NEW  SimplyEngaged program, where you’re able to
         earn up to $200 for completing health and wellness activities (refer to page 11).

         DENTAL is renewing with Humana with no cost or benefit changes. Our Buy-Up Dental plan has UNLIMITED
         annual maximum benefits and reimburses non-network dentists at the highest usual and customary level as
         well.  The Base Plan is best when you use in-network providers and has lower cost and benefit levels.

         VISION is offered through Superior Vision; renewed with no cost or benefit changes.

         VOLUNTARY LIFE and AD&D is offered through Mutual of Omaha for employees and dependents. August 1,
         2020  Annual  Open  Enrollment  and  NEW  HIRE  enrollment  after  August  are  Guaranteed  Issue,  NO  health
         questions  up  to  $150,000  for  employees,  $50,000  for  Spouses  and  $10,000  for  child(ren).  Voluntary  life
         insurance includes Accidental Death and Dismemberment (AD&D).

         VOLUNTARY STD and LTD. NEW this year with Mutual of Omaha is Low Cost Guaranteed Issue offering Short
         Term Disability starting 15th day with ability to add Long Term Disability starting after 90 days covering 60%
         of your Income Tax Free! LTD benefit period up to age 65 of Normal Social Security Retirement Age.

         Please retain a copy of this benefit guide for a brief review of your employee benefits, helpful information,
         costs, and necessary contact information. Our benefit guide will not serve as an ID card or verification of your
         benefits at a provider’s office, but it can provide assistance if needed.
