Secure Student Activity Permit Form at the DSSD
Submit the accomplished form to the following for Endorsement and
Approval:
A.USSG
B.DSSD
C.Campus Director
D.Director, General Services
Copy furnished the following offices of the Approved Activity Permit:
•DSSD
•Campus Director
•Security Guard
Submit report of the activity to the DSSD within Seven (7) days after
the activity.
