                     Student Activities- supervision, recognition and monitoring of
              student organizations and their activities such as leadership programs,
              student publication, student organizations, sports development,
              volunteerism, peer helper programs, etc.\

                             Secure Student Activity Permit Form at the DSSD









                    Submit the accomplished form to the following for Endorsement and
                  Approval:

                    A.USSG
                    B.DSSD
                    C.Campus Director
                    D.Director, General Services









                  Copy furnished the following offices of the Approved Activity Permit:

                    •DSSD
                    •Campus Director
                    •Security Guard









                    Submit report of the activity to the DSSD within Seven (7) days after
                    the activity.
