POLICIES ON CONFIDENTIALITY OF STUDENT RECORDS
Confidentiality is an essential part of the counseling relationship. A client/student must be able
to trust that the personal information he or she shares with a counselor will not be revealed to
a third-party, a counselor must explain the benefits and problems inherent in counseling
services and clarify the limits of confidentiality to the client.
Protect Client/Student Records:
1.To maintain confidentiality, it is the counselor's responsibility to keep the client's records
safe and appropriately secured.
2.Records are housed in a secure location that can be locked when the office is closed.
3.In most circumstances, a client's records are only to be released to a third party if the
client has provided written consent. When a subpoena is issued for a client's/student's
records, it is the counselor's responsibility to release them without the client's consent.
4.A student/client may request his or her own records in most situations.Some facilities
may require a meeting of attending counselors to determine if all or parts of the
client's/student's records should be released to him or her. The decision not to release
all parts of the record to a client may be made if the client is not emotionally and
mentally stable enough to cope with the information in the chart.
