Basic Life and AD&D Insurance — Protect Your Future
Life Insurance provides financial security for the people who depend on you. ABC Company provides Basic Life and Accidental Death and Dismemberment (AD&D) plus the opportunity to purchase Supplemental Life, Spouse and Child Insurance.
ABC Company provides Basic Life Insurance and AD&D coverage in the amount of 2x your base salary to a maximum
of $500,000 at no cost to you.

Supplemental Life Insurance coverage provides you and your family with additional financial security. Life Insurance is about more than paying for memorial services- it’s about making sure your family can maintain its standard of living over the long run if something happens to you. How much your family needs depends on your personal situation (other income, monthly expenses, and short-and long-term debt).
For open enrollment, your options available are:
» Yourself: Increase coverage by $10,000*, reduce existing coverage or waive. Coverage can not exceed $200,000 without submitting EOI
» Your Spouse: Reduce existing coverage or waive » Your Child(ren): Reduce existing coverage or waive
ENROLLING NOW HAS IT’S BENEFITS!
At any time, you will be able to elect Supplemental Life Insurance up to $200,000 for you, up to $25,000 for your spouse, and $10,000 for your qualifying dependents with proof of good health, or evidence of insurability (EOI).
