Health And Welfare Eligibility
You have 90 days from date of hire to enroll for coverage.
Who is eligible to enroll in health and welfare coverage?
Employees who work an average of 30 hours a week or more. The following employees’ dependents:
• Spouse
• Domestic Partner
• Your children, stepchildren or domestic partner’s children under the age of 26
When do I need to enroll in the health and welfare coverage?
As soon as you receive your first paycheck is the perfect time to enroll. You must enroll within your first 90 days of employment or your elections, with the exception of company paid life and LTD insurance, will default to waive. Your next opportunity to enroll won’t be until 2018 Benefits Open Enrollment, unless you experience a qualifying life event such as marriage, divorce, or the birth, adoption or placement for adoption of a child. You have 31 days to notify Human Resources of the
event, change your benefit elections and have your employee contributions
adjusted accordingly.
Benefits enrollment is online via ADP Self Service. Go to: https:// workforcenow.adp.com to enroll in benefits, view your paychecks and more online 24 hours a day, 7 days a week. First time registration passcode is DWIZ-1234.
Enrollment must be done on a computer and NOT a mobile device/smartphone.
