Why shifted towards more active engagement
in disease management? or clinical role?
• Increased prevalence of chronic diseases (non-
communicable diseases) and their co-
morbidities, which in turn leads to
polypharmacy which increases the risks of non-
adherence and drug-related problems (DRP).
• Both non-adherence and DRP result in poor
health outcomes and increased healthcare
costs (hospital admissions).It is estimated that
half of these hospitalisations is preventable .

What can pharmacists do?
3. Extemporaneous preparation and small-scale
manufacture of medicines
Pharmacists everywhere continue to prepare medicines in
the pharmacy.
This enables them to adapt the formulation of a medicine
to the needs of an individual patient. (pediatrics, topical,
respiratory..)
