If you have been assaulted or had an accident at school

Fill out the document in case of an event

Submit the original to management. Keep a copy!
The report must be completed and submitted to the
administration before leaving school, on the same day!

Don't hesitate to see a doctor.
Question: Following the incident, I was fine, but two days
later, the pain started. Can I see a doctor?
Answer: As long as the event occurrence report form is filled
out, you are protected. You can consult either the same day
or a few days later.
The doctor gives you a sick leave.
Send the medical certificate to your employer.
Fill out the CNSST form: Employer's Notice - Reimbursement
Request. Online form available on the CNSST website.
Reminder
An assault is not just physical; a threat or an insult also
constitutes an assault!
