Dear Molly,

We at Colonial Life are proud to serve you and your employees. By expanding the benefits choices, you
have enhanced your company’s benefit program by meeting the individual needs of your employees.

Enclosed is an administrative service guide, brochures and other information about your enrollment; The
first payroll deduction will be on 07/17/2020, second 07/31/2020, third 08/14/2020.

When you receive your Colonial bill from this enrollment, some employees may not be processed
through our underwriting department and therefore will be missing from the bill. As underwriting
completes the processing and the policy is issued, all premium collected will be credited to the
employee. If an employee should leave your employ, simply log into the online web portal
www.coloniallife.com to remove the employee or cross them off your billing statement and deduct
their premium from the balance due. This will reconcile your statement to your payroll deductions
and also inform our Home Office to send out a letter to the former employee asking if they would
like to continue their Colonial Coverage.
