Millie is served by dietary and nursing shift staff, working
in traditional, hierarchal structures. Management makes decisions and Millie
is told “how we do it here” by staff who learned what
to do through inservices
and procedures. Millie’s dining is governed by clinical needs such as med pass, therapy schedules, and staff scheduling. In a progressive traditional home, special teams or committees may
be formed and dedicated to changing the environment or service delivery to better meet resident needs, but their scope is limited.
Consistent, permanently assigned staff serve Millie, forming relationships and getting to know her preferences for dining. The opinions
and knowledge of these care team members is solicited by management before decisions are made, so Millie has a
voice through her care team members. Clinical issues and licensed nursing staff make most decisions and form the care giving team, but consistent dietary and housekeeping staff may also work closely with the team.
Millie’s neighborhood is
led by a non-clinical leader who coordinates the team
of care givers on all non- clinical issues. Millie’s team is consistent, and is empowered to problem solve, and plan specific activities, dining and social events to best meet Millie’s needs and wishes. Her care team can generally self-schedules, and the schedule may vary from those of another neighborhood so that residents’ needs can be best met.
A self directed team leads Millie’s household. Its members are cross trained so all can serve Millie’s universal needs, and meeting those needs is everyone’s job. Her household team is responsible for planning and controlling the work in her household, so Millie’s needs are close to the decision-makers, and their decisions are based on those needs.
