Goals That Belong to the Person
When caregivers record residents’ stated goals on their care plans either in the third person (“Pat states her goal for
her life now is...”) or in the resident’s voice (“I want to be active daily,”) it takes pressure off staff. They are rightfully the residents’ goals, not ours.
But what if a resident cannot tell us his goals? It seems reasonable that a family member, friend or other responsible party involved in the resident’s life would be asked to try to answer on his behalf. However, such a person does not always exist. So then it follows that the goal becomes what we call “staff determined.” In other words, using all we know from observing and caring
for the person, we often do know what the resident would tell us is their goal even though they cannot. So think in three tiers: resident-determined, family- determined and staff determined.
