How to Use This Workbook
Steering Team:

We hope you find this workbook and video enlightening. The workbook has been designed to accompany the video, A Tale of Transformation; Four Stages Tell The Story. Our primary purpose in providing the workbook is to offer many points of stimulation for individuals and teams working to change the culture of their homes and to move toward resident-directed care.
We hope that you find the combination of video and workbook to be of great value in your journey. Please take charge of your culture change progress and remember that there is no set chronology of actions that your organization must take. Instead, the one absolute that must be built into your journey is the High Involvement of all staff and residents in talking about and mapping the organization’s progress. Though new to long term care, this successful approach of involvement has been an integral part of culture change innovations in other industries. (See website resources in the back of this workbook).
The side column of each page in this workbook refers to excerpts and images from the video and follows the actual story line of the video. The main content offers thought provoking comments and questions, exercises that an individual or a team can do, resources and links to relevant materials, and next steps that an organization can take to progress on the journey of culture change.
There are many ways to use the workbook. You may find it most helpful to have a small team (perhaps the Steering Team or an Education or High-Involvement team) to study the entire workbook and video, and then decide what segments are relevant for any particular team, work session or training segment.
You may wish for your Steering Team (or Leadership Team of formal and informal leaders) to work their way through the video and workbook chronologically. Because the video is 66 minutes long, you should plan to do your study and discussion over a number of sessions.
The accompanying DVD is set up in segments (by chapter, and also with and without summary graphics) and can be easily accessed to demonstrate a single point, or to highlight an issue that needs to be discussed and addressed in your organization.
