whenever the need for leave is foreseeable. When the need for such leave is not foreseeable 30 days in advance, the associate must give as much notice as possible, based on the circumstances.
Requests for General Medical Leave must be accompanied by a Medical Certification Form (available from the HR Department).
Properly supported and timely requests for General Medical Leave from eligible associates will be granted as accommodations are possible.
LEAVE OF ABSENCE
An unpaid Leave of Absence (LOA), if not detrimental to the services provided by The Piper or the welfare of other staff members affected, may be granted under circumstances described in this policy. LOA requests that involve issues controlled by other personnel policies are governed by this policy and the other controlling policy. For example, a request for leave because of pregnancy, childbirth, adoption, or illness of a child, spouse or parent would be governed by both the Family and Medical Leave Policy and the LOA Policy.
LOA requests should be made before weekly schedules are finalized and before the associate exhausts applicable paid leave. In general, associates should request LOA as far in advance as possible. Exceptions to these notice requirements may be made when emergency or sudden illness conditions exist.
Paid leave of any type does not accrue when an associate is on LOA for more than fifteen (15) calendar days.
LOA is usually used for (but not limited to) the following reasons:
• Time off for associates who do not accrue paid leave (PTO).
• Extended travel (request must be submitted at least 60 days in advance).
• Prolonged illness of an associate or associate’s family member.
• Funerals
• Moving
• Marriage
• Emergencies or crises, which require absence from work
Continuation of Benefits during Leave
While on a leave of absence provided for under these policies, we will continue your group health insurancebenefitsunderthesametermsasprovidedtootherassociates,for uptoamaximumof16 weeks leave time during any one-year period. You will be responsible for any associate contribution toward insurance while on leave.
If your leave extends beyond 16 weeks, you shall be offered the opportunity to purchase continuing coverage under state and federal COBRA continuation rules.
NO FAULT ATTENDANCE POLICY
Action Pact seeks to employ reliable and dependable associates and expects associates to report to work for their scheduled shift at the scheduled time and to work all scheduled hours (including scheduled
overtime). Reliable and punctual attendance is essential to facilitate the orderly and efficient operation of each location and to ensure quality resident care. Excessive tardiness and absenteeism increases the workload of one’s
