Employee has been hired and credentialed by a UT Southwestern affiliated institution (i.e. have a user ID issued by UT Southwestern, Children’s or Parkland)
Research team member(s) handling eResearch systems have requested eResearch system access for both VELOS and eIRB.
eResearch access has been obtained from UT Southwestern Academic Information Systems (AIS)
You Have a protocol for Human Subject’s Research and appropriate documents (e.g. Informed Consent documents, HIPAA, questionnaires, surveys, advertisements)
