ii) Click the consent form link in question 4.4. This will take you to the modified study smartform. In the parent protocol modified study consent form to section 54.1.1a. If a consent form was loaded initially, then you will click the upload revision link instead of add.
iii) Click save, then exit to get out of the parent protocol smartform and return to the modification form.
iv) Back in the modification form, click the continue button to proceed.
v) Answer the questions in the form, as applicable. For this example, the modification also includes changes to the study personnel, so section 11 is shown. Add and remove the team members, as appropriate, and click the Continue button to proceed.
