iii. The Continuing Review associated with the study is listed, along with the state of the Continuing review.
iv. Once the continuing review has been approved by the IRB, the PI, coordinator/associate, and primary administrative contact will be notified.
v. All consent and HIPPA forms will be updated with the new expiration date. Print these out and add to your regulatory binder. Mark a line through the previous iteration to avoid confusion.
