On-site Research Sponsor Representative Requirements
The following instructions must be followed for all on-site research sponsor representative visits. If you have questions, please contact Research Education.
Requirements for research sponsor representatives visiting a Children’s HealthSM campus:
• Sign in with a valid appointment and current credentials.
• Complete screening with concierge upon entering the facility. The concierge will run the representatives
license through the FastPass system and provide a visitor badge.

monitors will need to be taken to the main hospital to be checked in and receive a visitor badge.
• Be escorted by research staff at all times.
Requirements for research sponsor representatives intending to visit any clinical/patient care area(s):
• Register with Vendormate prior to their visit.
• Check-in at the Vendormate kiosk and receive clearance.
• Be compliant with all documentation, policies and vaccination, requirements.
Effective Oct. 1, all on-site visitors must be fully vaccinated against COVID-19
When requesting on-site visit approval, please complete the required sponsor representative COVID-19 vaccine attestation and email it to Deborah.Town@childrens.com. On-site visits will not be approved without the attestation. Research sponsor representatives who have already provided an attestation do not have to submit a new form for each visit unless their status changes (i.e., if they are delinquent on any required booster per federal guidance).
Required on-site visit request process:
Submit the following to Deborah.Town@childrens.com for all on-site visit requests, once reviewed and approved you will be notified by email. The following details must be provided for an on-site sponsor representative/monitor visit:

• Study Title:
• IRB#:
• Velos#:
• Date of on-site visit:
• Purpose for on-site visit:
• Meeting room location:
• Children’s Health campus to be visited:
• Locations to be visited on-site (e.g., Pharmacy, lab, etc.):
• Names and roles of those who need to be physically present in the meeting:
• Include a copy of the required sponsor representative COVID-19 vaccine attestation
