11) Signature and Approval: The employee will provide their signature and date of signature signifying that they agree and confirm the terms of the job description.
3. SCOPE
Job descriptions are expected to be completed and kept on file for all Children’s Health Employees. The current SOP is applicable to all employees of Children’s Health and their direct supervisor.
4. RESPONSIBILITIES
4.1 Maintenance & Approval
As referenced in CHST policy 2.06 Verification and Evaluation of Competence of Personnel, all job descriptions are defined by Research Administration management and approved by HR. Maintaining relevant job descriptions are managed by Human Resources (HR) and the department through annual review for their applicability. Job descriptions are reviewed in reference to relevant protocol and documentation of the employee’s fulfilment of their job requirements and important contributions as documented on their annual job performance review, the Performance Excellence and Accountability for Children’s Health (PEAC) form. Research Administration management is responsible for reviewing all relevant practice, acts, rules and regulations as they pertain to the job description. Appropriate updates may be made to a job description as determined necessary.

4.2 Confirmation of Agreement
All potential employees are considered for employment based upon the required hiring competencies. The ability to perform specific knowledge, skills, and abilities is demonstrated through the hiring process, on-boarding, and ongoing training for initial and ongoing competencies, as well as for study specific requirements of the sponsor. Upon hire and as part of the on-boarding process, the job description is reviewed with the new employee by the supervisor and the employee is given a chance to ask questions and to confirm the expectations of their job position. A formal agreement is made by the employee’s signature and date of signature. The supervisor also provides their signature and date of signature. Any changes in the job description or an employee’s job position require a new confirmation of agreement indicated by the employee’s signature and date on the updated/new job description.
4.3 Record on File
The employee’s supervisor should keep a copy of the job description along with all necessary hiring competency documentation including licences; certifications; registrations; training certificates; education records; peer, patient/family, and physicians evaluations; and PEAC reviews. Additionally, a signed copy of the job description should be provided to the employee and to HR. HR also requires coaching notes, the department orientation checklist, education records, interview notes, and traveler’s agency competencies. It is recommended that copies of competency
