f. Repeat this process on a monthly basis, or in a manner that supports the needs of the study throughout the duration of the study.
5.1 Preparing the Lab Kit for a Study Visit How to Breakdown a Lab Kit:
When preparing for a visit, separate all the collection tubes/cups along with the sponsor’s lab requisition sheet and specimen bag as follows:
For Example:
Specific Supplies in each Lab bag/kit:
• Labeled Sample tube(s)
• Labeled Transport tube(s), if needed.
• All kit supplies (diff - stick slides, etc.)
• Children’s Health lab Instruction Sheet
• Sponsors lab requisition sheet (provided from sponsor in lab kit)
Creating Lab Kit/Bag:
Each collection tube/cup will be in a separate specimen bag along with a Children’s Health lab instruction sheet created by the Research Coordinator. Please, follow the following steps:
1. Refer to the lab Instruction sheet to identify the correct tube and supplies for each sample.
o Forexample,the“chemistry”bloodsamplerequiresa4-mllavender-top vacutainer tube, and a red-top transfer tube.
