2. Apply the appropriate label on each tube and write the subject number, along with visit title and complete the label as defined in the lab manual or protocol.
o Continuing with the previous example, place the sponsor provided “Chemistry” label on the red-top, transfer tube and write the subject ID number and date of visit on the label for the chemistry sample. Please, refer to the sponsor specific instructions for labelling tubes.
3. Once, the sample is properly labelled. Place all contents pertaining to a particular sample in a separate lab specimen bag.
o Continuing with the previous example, place the 4-ml lavender-top, vacutainer tube along with the red-top, transfer tube in the specimen bag for the chemistry sample.
4. On the Children’s lab Instruction sheet, identify the visit, visit date, and complete the collection time upon collecting the sample.
o Continuingwiththepreviousexample,writethevisitnumber(e.g.,Visit 2), the visit date 12/15/2016, and complete the time of collection at the study visit.
5. Complete steps 1-4 for the all of the samples to be collected for the particular study visit.
For Example:
The picture below shows , the collection tube in a specimen collection bag along with the Children’s Health lab Instruction sheet.
Each sample should be inside its own bag with sponsor provided labels. Please do not mix samples and transport tubes together.
